Alfa Medical Team s.r.o. is responsible for the processing of personal data in accordance with Regulation 2016/679 of the European Parliament and the EU Council of 27 April 2016 on the protection of natural persons in the processing of personal data and on the free movement of such data (hereinafter referred to as GDPR) and pursuant to Act No. 18/2018 Z . of.
We process the personal data of our customers only to the extent necessary for the correct processing of the order, issuing the invoice and delivering the goods. Our company does not provide this data to a third party, except for the necessary data for the accounting company and the forwarding company that ensures the delivery of the goods to you. The customer has the right to have the provided data deleted at any time after sending the request by e-mail.
- Affected persons (contracting parties): buyer (customer) and operator of the online store (Alfa Medical Team, s.r.o., with registered office Priemyselná 6, 917 05 Trnava, ID: 46841148)
- List of personal data processed in the order process: first name, last name, country, street, zip code, city, telephone contact, e-mail address.
- Purposes of personal data processing: issuance of a tax document, contacting the customer regarding the order, fulfillment of the contract. The processing of personal data is necessary for the performance of a contract to which the customer (person concerned) is a party or to take measures prior to the conclusion of the contract based on the request (order) of the customer (person concerned). Personal data must not be further processed in a way that is not compatible with these purposes.
- Recipients: accounting company, forwarding company.
- Legal basis for processing personal data: a) Processing of personal data (first name, surname, country, street, zip code, city) is necessary according to a special regulation or an international treaty to which the Slovak Republic is bound. Above all, according to Act no. 222/2004 Coll. on value added tax. In the event that the customer (person concerned) does not provide the operator of the online store (Alfa Medical Team, s.r.o.) with personal data to the extent specified, the operator cannot conclude a contract with him. b) The processing of personal data (e-mail, telephone contact) is necessary for the performance of the contract.
- Period of storage of personal data: is governed by the obligation to archive accounting documents and is 10 years, primarily according to Act no. 431/2002 Coll. on accounting and Act No. 222/2004 Coll. on value added tax.
By agreeing to the terms and conditions, the customer (person concerned) gives his express and unreserved consent for the operator of the online store (Alfa Medical Team, s.r.o.) to process his personal data for the above-mentioned purposes, during the above-mentioned period and in the above-mentioned scope. The customer (data subject) has the right to withdraw his consent at any time. Withdrawal of consent does not affect the legality of processing based on consent before its withdrawal. The customer (person concerned) is obliged to provide true and up-to-date personal data.
The customer (the affected person) has the right to file a complaint with the supervisory authority, the right to object to the processing of personal data, the right to demand from the operator of the online store (Alfa Medical Team, s.r.o.) access to personal data concerning him (the affected person), the right to correction or deletion or restriction of processing of personal data, as well as the right to data portability.
